If you are involved in an accident / incident please follow these steps

1. Take pictures/video of accident/ incident scene, any damage or injuries sustained as a result of accident/ incident, pictures of driver license of driver or resident, insurance and tag if involved in vehicle accident.
2. Call 911 and get copy of police report and bring to dispatch
3. Call (800) 942-3300 to get LMET#
4. Fill out incident report with civilian name, address, phone number and email address.
5. Attach pictures / video of scene by clicking “Attach Photos”
6. Contact Dispatch 1(772) 979-5954
7. click SUBMIT REPORT after all steps have been completed
